First-time patient appointments are scheduled with a nurse practitioner for a get acquainted visit.
We set aside a half-hour appointment to assess your individual needs and develope an effective plan of care.
After your first appointment you can feel free to schedule with any provider that you would like to see.
What can I expect for my first appointment?
When is my co-pay due?
Co-pays are due in full at the time of service unless other previous arrangements have
been made. We will gladly accept payments made by credit/debit card, check, and cash.
What are your procedures for checking in?
Due to constant changes in insurance plans we will ask you for your insurance card upon every appointment.
This helps us to keep your information current and minimize any delay that there may be in getting coverage
for your visits. Our HIPAA forms include a space for you to list who may have access to your medical
information or speak on your behalf. Updated HIPAA forms are collected at least once a year. We also hand
out optional consent forms to communicate with you via e-mail. Our check-in procedures are in place to
better serve you as well as keeping current with compliance requirements.

I’m a new patient, can I register online?
Yes! Call 260.480.2600 for the registration password then click the “New Patient” button below.
WebView is a patient and physician portal service that allows authorized physicians and patients the ability to
view patient records over the internet in a secured and confidential manner. WebView also allows patients to
register, complete patient intake forms, and request appointments. Our office is very excited to implement
this new portal to our patients!
When sending messages to the office through WebView, you have the option of messaging 4 different message
boxes.
