* Payment is expected at the time of service unless prior arrangements have been made. We try to work with everyone. We file your insurance for you but will have to collect Co-Pays, Deductibles and non-covered service fees.

* You will need to present your insurance card at each visit in order to file your insurance.

* Fees will be charged for missed visits unless you have called to cancel prior to the visit.
